How to renew membership for July 1 2010 - June 30 2011


Please choose one of the following options:

  1. Download the renewal form to your computer, save it as a word document, fill in your details and add your scanned signature. Then email it back to office@capacar.org.au.

  2. Download the renewal form to your computer, and print it out. Fax the completed and signed form to 02 6262 9993 or mail it to CAPACAR Office P O Box 12 Deakin West ACT 2600.

  3. Download the renewal form to your computer, print it out, fill it in, sign it, then scan the completed form into your computer and email it to office@capacar.org.au.

Remember to include the record of Supervision hours form if you are a Clinical Member. Contact the CAPACAR office if you have any queries.
